RESUMO

Hostile aortic neck anatomy challenges the outcomes of endovascular abdominal aortic aneurysm repair (EVAR). Besides reverting to open surgical repair (OSR), thoughtful endograft selection and a number of advanced endovascular techniques have been suggested as potential solutions for preventing proximal seal zone complications, improving EVAR durability, and preventing aneurysm-related death. Each technique is associated with advantages and limitations and there has not been a credible direct comparison amongst them in the form of a well-designed prospective trial. The not infrequent presence of multiple hostile anatomic characteristics further complicates decision making and challenges the surgeon's skills. This paper serves as an overview of hostile neck anatomy and its implications on EVAR. We provide a concise literature review with the purpose of outlining the treatment modalities and outcomes in this patient population.

Protected areas are of paramount relevance to conserving wildlife and ecosystem contributions to people. Yet, their conservation success is increasingly threatened by human activities including habitat loss, climate change, pollution, and species overexploitation. Thus, understanding the underlying and proximate drivers of anthropogenic threats is urgently needed to improve protected areas' effectiveness, especially in the biodiversity-rich tropics. We addressed this issue by analyzing expert-provided data on long-term biodiversity change (last three decades) over 14 biosphere reserves from the Mesoamerican Biodiversity Hotspot. Using multivariate analyses and structural equation modeling, we tested the influence of major socioeconomic drivers (demographic, economic, and political factors), spatial indicators of human activities (agriculture expansion and road extension), and forest landscape modifications (forest loss and isolation) as drivers of biodiversity change. We uncovered a significant proliferation of disturbance-tolerant guilds and the loss or decline of disturbance-sensitive guilds within reserves causing a "winner and loser" species replacement over time. Guild change was directly related to forest spatial changes promoted by the expansion of agriculture and roads within reserves. High human population density and low nonfarming occupation were identified as the main underlying drivers of biodiversity change. Our findings suggest that to mitigate anthropogenic threats to biodiversity within biosphere reserves, fostering human population well-being via sustainable, nonfarming livelihood opportunities around reserves is imperative.

BACKGROUND: Perfluorocarbon (PFC)possesses unique chemical properties that favor the pigment epithelium's adhesion and allows the drainage of subretinal fluid through retinal holes present in retinal detachment cases. However, PFC as a temporary tamponade agent has been limited due to its high potential for toxicity. MAIN BODY: We conducted a scoping review regarding the use of PFC in vitreoretinal surgery as a temporary tamponade in subjects with severe ocular trauma or severe retinal detachment who received a therapeutic intervention (vitrectomy via posterior approach with the use of PFC as a temporary tamponade), compared to vitrectomy without the use of PFC as a temporary tamponade. Outcomes of interest were retinal reattachment, visual acuity (VA), postoperative complications and retinal toxicity. The search was performed in Medline, Medline In-Process & Other Non-Indexed Citations, Medline Daily Update, Embase databases. Reference lists from relevant review articles were also included. Two hundred thirty-eight studies were found, with no duplicate entries. In the first selection, 230 articles were eliminated; in the second selection, 6 additional articles were discarded. In total, 8 articles were obtained in this review. Two selected articles corresponded to animal studies and 6 to studies in humans. Regarding study design, 5 were case series, and 1 was a cohort study. CONCLUSION: PFC as a short-term tamponade had high rates of reapplication, improved VA, and the most frequent adverse effects were reversible after PFC withdrawal. Nonetheless, the quality of the studies was poor. Studies with more rigorous methodologies are needed to determine visual and structural outcomes and potential risks of PFC use as a temporary tamponade in vitreoretinal surgery.

Congenital heart disease (CHD) is a common structural anomaly, affecting ~ 1% of live births worldwide. Advancements in medical and surgical management have significantly improved survival for children with CHD, however, extracardiac malformations (ECM) continue to be a significant cause of morbidity and mortality. Despite clinical significance, there is limited literature available on ECM in neonates with CHD, especially from Latin America. A cross-sectional study of neonates with severe CHD evaluated by the medical-surgical board team at Fundación Cardiovascular de Colombia from 2014 to 2019 was completed to characterize morbidity, mortality, surgical outcomes, and ECM. Demographics and surgical outcomes were compared between neonates with and without ECM. Medical record data were abstracted and descriptive statistical analysis was performed. Of 378 neonates with CHD, 262 had isolated CHD (69.3%) and 116 had ECM (30.7%). The most common ECM was gastrointestinal (n = 18, 15.5%) followed by central nervous system (n = 14, 12%). Most neonates required a biventricular surgical approach (n = 220, 58.2%). Genetic testing was performed more often for neonates with ECM (n = 65, 56%) than neonates with isolated CHD (n = 14, 5.3%). Neonates with ECM had lower birth weight, longer hospital stays, and higher postsurgical complications rates. There was no difference in survival between groups. Overall, Screening for ECM in neonates with CHD is important and identification of ECM can guide clinical decision-making. These findings have important implications for pediatric healthcare providers, especially in low- and middle-income countries, where the burden of CHD is high and resources for managing CHD and extracardiac malformations may be limited.

RESUMEN Introducción: la preeclampsia (PE) es la principal causa de morbimortalidad materno-fetal en nuestro país. Alteraciones hemodinámicas precoces durante el embarazo podrían predecir la evolución a PE. El machine learning (ML) permite el hallazgo de patrones ocultos que podrían detectar precozmente el desarrollo de PE. Objetivos: desarrollar un árbol de clasificación con variables de hemodinamia no invasiva para predecir precozmente desarrollo de PE. Material y métodos: estudio observacional prospectivo con embarazadas de alto riesgo (n=1155) derivadas del servicio de Obstetricia desde enero 2016 a octubre 2022 para el muestreo de entrenamiento por ML con árbol de clasificación j48. Se seleccionaron 112 embarazadas entre semanas 10 a 16, sin tratamiento farmacológico y que completaron el seguimiento con el término de su embarazo con evento final combinado (PE): preeclampsia, eclampsia y síndrome HELLP. Se evaluaron simultáneamente con cardiografía de impedancia y velocidad de onda del pulso y con monitoreo ambulatorio de presión arterial de 24 hs (MAPA). Resultados: presentaron PE 17 pacientes (15,18%). Se generó un árbol de clasificación predictivo con las siguientes variables: índice de complacencia arterial (ICA), índice cardíaco (IC), índice de trabajo sistólico (ITS), cociente de tiempos eyectivos (CTE), índice de Heather (IH). Se clasificaron correctamente el 93,75%; coeficiente Kappa 0,70, valor predictivo positivo (VPP) 0,94 y negativo (VPN) 0,35. Precisión 0,94, área bajo la curva ROC 0,93. Conclusión: las variables ICA, IC, ITS, CTE e IH predijeron en nuestra muestra el desarrollo de PE con excelente discriminación y precisión, de forma precoz, no invasiva, segura y con bajo costo.


ABSTRACT Background: Preeclampsia (PE) is the main cause of maternal-fetal morbidity and mortality in our country. Early hemodynamic changes during pregnancy could predict progression to PE. Machine learning (ML) enables the discovery of hidden patterns that could early detect PE development. Objectives: The aim of this study was to build a classification tree with non-invasive hemodynamic variables for the early prediction of PE occurrence. Results: Seventeen patients (15.18%) presented PE. A predictive classification tree was generated with arterial compliance index (ACI), cardiac index (CI), cardiac work index (CWI), ejective time ratio (ETR), and Heather index (HI). A total of 93.75% patients were correctly classified (Kappa 0.70, positive predictive value 0.94 and negative predictive value 0.35; accuracy 0.94, and area under the ROC curve 0.93). Conclusion: ACI, CI, CWI, ETR and HI variables predicted the early development of PE in our sample with excellent discrimination and accuracy, non-invasively, safely and at low cost.

Introducción: Los bosques de niebla se caracterizan por su distribución insular en sistemas montañosos tropicales. Florísticamente son complejos y heterogéneos, y con vacíos de información que afectan la comprensión de cómo varía su diversidad y estructura. Objetivo: Analizar los patrones de diversidad, estructura y composición del bosque de niebla en México. Métodos: Contamos y medimos plantas leñosas en 40 parcelas de 0.1 ha de cinco sitios en aproximadamente 200 km a lo largo de la Sierra Madre de Chiapas, México (1 700 - 2 100 m.s.n.m.). Usamos tres métodos estadísticos: análisis de varianza (diversidad verdadera y estructura entre sitios), regresión simple y métodos multivariados (clima y elevación). Resultados: Registramos 4 021 individuos (220 especies, 60 familias). En la diversidad verdadera, solo encontramos diferencias en riqueza de especies, con un ligero incremento hacia la parte central del área. La diversidad fue constante para especies comunes y dominantes. No encontramos diferencias en la densidad de árboles o área basal entre los sitios. Sin embargo, hubo menor densidad y mayor área basal a mayores elevaciones. La diversidad beta y la diferenciación en la composición florística es alta e importante entre las parcelas de un mismo sitio y se incrementa con la distancia entre los sitios. Conclusiones: La diversidad beta y la diferenciación florística contribuyen significativamente en la variación del bosque de niebla. Las variables climáticas y la altitud tienen efectos distintos sobre la diversidad y estructura.


Introduction: Cloud forests are noted for their narrow distribution in tropical mountain systems. Floristically, they are complex and heterogeneous, with gaps in information that hinder the understanding of how their diversity and structure vary. Objective: To analyze patterns of diversity, structure, and composition of the cloud forest in Mexico. Methods: We counted and measured woody plants in forty 0.1 ha plots from five sites in approximately 200 km along the Sierra Madre of Chiapas, Mexico (1 700 - 2 100 m.a.s.l.). We used three statistical methods: analysis of variance (diversity true and structure among sites), simple regression and multivariate statistics (climate and elevation). Results: We recorded 4 021 individuals (220 species, 60 families). In true diversity, we only found differences for species richness, with a slight increase toward the central part of the area. Diversity was constant for common and dominant species. We found no differences in tree density or basal area between sites. However, there was less density and more basal area at higher elevations. Beta diversity and differentiation in floristic composition are high and important between plots of the same site, and they increase with distance between sites. Conclusions: Beta diversity and floristic differentiation significantly contribute to variation in the cloud forest. Climatic variables and altitude have different effects on diversity and structure.

RESUMO

We present the case of a 59-year-old male who sustained an ascending aortic injury and a subdural hematoma after a head on collision. After undergoing emergent craniotomy for evacuation of the subdural hematoma, he was maintained with strict blood pressure control. Once able to be safely anticoagulated, he underwent replacement of the ascending aorta. This exceedingly rare case was managed by a multidisciplinary team approach that led to an optimal outcome given his disastrous multi-traumatic injuries.

OBJECTIVE: To identify corneal structure differences on quantitative high-frequency ultrasound biomicroscopy (UBM) among subjects with congenital glaucoma compared with controls. METHODS: This prospective case-control study evaluated 180 UBM images from 44 eyes of 30 subjects (18 control and 12 glaucoma, mean age 5.2±8.0 years, range 0.2-25.8 years) enrolled in the Pediatric Anterior Segment Imaging and Innovation Study (PASIIS). ImageJ was used to quantify a comprehensive set of corneal structures according to 21 quantitative parameters. Statistical analysis compared corneal measurements in glaucoma subtypes and age-matched controls with significance testing and mixed effects models. RESULTS: Significant differences between congenital glaucoma cases and controls were identified in 16 of 21 measured parameters including angle-to-angle, central and peripheral corneal thicknesses, scleral integrated pixel density, anterior corneal radius of curvature, and posterior corneal radius of curvature. Eight parameters differed significantly between primary congenital glaucoma and glaucoma following congenital cataract surgery. CONCLUSION: Multiple measurable corneal structural differences exist between congenital glaucoma and control eyes, and between primary and secondary congenital glaucoma, including but not limited to corneal width and thickness. The structural differences can be quantified from UBM image analysis. Further studies are needed to determine whether corneal features associated with glaucoma can be used to diagnose or monitor progression of congenital glaucoma.

AIMS: To evaluate arterial stiffness indicators in people with prediabetes (PreD) and its possible pathogenesis. MATERIALS AND METHODS: Pulse wave velocity (PWV) was measured in 208 people with FINDRISC ≥ 13 (57 ± 8 years old, 68.7% women) and thereafter divided into those having either normal glucose tolerance (NGT) or PreD. In each subgroup we also identified those with/out insulin resistance (IR) measured by the triglyceride/HDL-c ratio (normal cut off values previously established in our population). Clinical and metabolic data were collected for all participants. PWV was compared between subgroups using independent t test. RESULTS: Women and men had comparable clinical and metabolic characteristics with obesity (BMI ≥ 30) and antihypertensive-statin treatment, almost half with either NGT or PreD. Whereas 48% of NGT people presented IR (abnormally high TG/HDL-c ratio), 52% had PreD. PWV was significantly higher only in those with a complete picture of metabolic syndrome (MS). CONCLUSIONS: Since PWV was significantly impaired in people with a complete picture of MS, clinicians must carefully search for early diagnosis of this condition and prescribe a healthy life-style to prevent development/progression of CVD. This proactive attitude would provide a cost-effective preventive strategy to avoid CVD's negative impact on patients' quality of life and on health systems due to their higher care costs.

The study was a prospective observational study comparing semiautomated to manual quantitative ultrasound biomicroscopy image analysis among 82 images from 41 eyes of 32 subjects (21 controls and 11 glaucoma) enrolled in the Pediatric Anterior Segment Imaging Innovation Study. Intraclass correlation coefficients and correlation coefficients were >0.8 for all parameters, and comparison of respective analysis speed was 7 times faster for the semiautomated method compared with manual image quantification.

Introducción El posicionamiento adecuado de la copa acetabular en el reemplazo total de cadera (RTC) puede mejorar la función, reducir el desgaste, el pinzamiento y la luxación. La posición de la copa se describe como la relación espacial entre el centro de rotación de la cadera y la pelvis. El objetivo de este estudio es describir el posicionamiento de una copa monobloque no cementada en el posoperatorio inmediato de los pacientes a quienes se les realizo un RTC primario. Materiales y Métodos Estudio observacional descriptivo, los pacientes fueron operados por 4 cirujanos especialistas en cirugía de reemplazo articular de cadera en 8 clínicas, los procedimientos se realizaron entre octubre de 2015 a agosto de 2018, las medidas radiológicas fueron la inclinación, anteversión, lateralización, porcentaje de descubrimiento y espacio detrás de la copa acetabular. Resultados Los mayores porcentajes son para las copas acetabulares con inclinación de 36 a 48 grados, anteversión de 5 a 15 grados, lateralización de -2 a +2mm, porcentaje de descubrimiento del 0% y espacio detrás de la copa de 0 a 3mm. Discusión Diferentes autores han identificado zonas seguras de implantación las cuales concuerdan con los resultados de nuestro estudio. El posicionamiento adecuado de la copa acetabular monobloque no cementada se pudo lograr con una correcta indicación del implante, un abordaje quirúrgico que permitió una ubicación tridimensional idónea y un acetábulo sin alteraciones anatómicas a nivel periférico.


Background Appropriate positioning of the acetabular cup in total hip arthroplasty (THA) can improve hip function, reduce wear, impingement and dislocation. The position of the cup is described as the spatial relationship between the centre of rotation of the hip and the pelvis. The aim of this study is to describe the positioning of the uncemented monoblock cup in the immediate postoperative period in patients who underwent a primary THA. Methods Descriptive observational study. The patients were operated by 4 joints arthroplasty surgeons in 8 different hospitals, from October 2015 to August 2018. The radiological measurements were taken from the AP Pelvis Xrays of the immediate postoperative. The inclination, anteversion, lateralization, percentage of cup uncovered and the space behind the acetabular cup. Results 82.05% of the cups were in the range of inclination from 36 to 48 degrees; 85.71% had an anteversion between 5 to 15 degrees, 76.56% presented a lateralization of -2 to +2mm, 94.51% of the patients didnt have a uncover cup, and 92.67% of the cups had a space behind the cup from 0 to 3mm. Discussion In our study the highest percentage of cups were implanted with parameters agree to the safety areas reported by various authors. Adequate positioning of the uncemented monoblock acetabular cup can be achieved with a proper indication, a surgical approach that allows an ideal three-dimensional location and an acetabulum with a good peripheral rim that provides the required press-fit.

Introducción: La vasculatura fetal persistente bilateral es una malformación ocular poco frecuente, generalmente unilateral, que suele presentarse como leucocoria. Objetivo: Presentar el caso de una paciente de 6 meses con leucocoria bilateral y diagnóstico de vasculatura fetal persistente bilateral. Resumen del caso: Reporte de caso por medio de recopilación de datos de la historia clínica, fotografías del segmento anterior de la paciente, ecografía ocular y resonancia magnética de cerebro. Conclusiones: La vasculatura fetal persistente bilateral es poco frecuente, pero hay que tenerla en cuenta en el momento del abordaje de un paciente con leucocoria bilateral, cuyo pronóstico visual depende de su reconocimiento y tratamiento tempranos.


Introduction: Bilateral persistent fetal vasculature is a rare developmental disorder of the eye, usually seen as leukocoria affecting only one eye. Objective: Report on a case of a 6 month old patient with bilateral leukocoria and diagnosis of bilateral persistent fetal vasculature. Case summary: Descriptive case report study with data from de medical history, anterior segment photographs, ocular ultrasonography and cerebral magnetic resonance imaging. Conclusions: Bilateral persistent fetal vasculature is an infrequent disease that has to be taken into account when approaching a patient with bilateral leukocoria, whose visual prognosis depends on the early detection and treatment

PURPOSE: To quantitatively describe the structural corneal changes from infancy to early adulthood using ultrasound biomicroscopy. METHODS: In this prospective study, 168 ultrasound biomicroscopy images were obtained from 24 healthy eyes of 24 patients who consented and enrolled in the Pediatric Anterior Segment Imaging Innovation Study. Their ages ranged from birth to 26 years. An established ultrasound biomicroscopy imaging protocol including seven views of one eye per patient were obtained and measured using ImageJ software (National Institutes of Health). Twelve corneal structural parameters were measured. Means were compared between younger and older groups. RESULTS: Among the 12 measured structures, 5 demonstrated statistically significant differences (P < .05) between patients younger than 1 year and patients older than 1 year. The mean values for corneal cross-sectional width and length, central corneal thickness, and radii of curvature (anterior and posterior) were significantly different in patients younger than 1 year. Curvature and limbus-to-limbus dimensions changed more dramatically than thickness and tissue density. When comparing the youngest to oldest subgroups, anterior curvature flattened (6.14 to 7.55 radius), posterior curvature flattened (5.53 to 6.72 radius), angle-to-angle distance increased (8.93 to 11.40 mm), and endothelial cross-sectional distance increased (10.63 to 13.61 mm). CONCLUSIONS: Pediatric corneal structures change with age. The most significant changes occur in the first months of life, with additional changes later in childhood. Purpose: Ultrasound biomicroscopy (UBM) is a noninvasive method for assessing anterior segment anatomy. Previous studies were prone to intergrader variability, lacked assessment of the lens-iris diaphragm, and excluded pediatric subjects. Lens status classification is an objective task applicable in pediatric and adult populations. We developed and validated a neural network to classify lens status from UBM images. Methods: Two hundred eighty-five UBM images were collected in the Pediatric Anterior Segment Imaging Innovation Study (PASIIS) from 80 eyes of 51 pediatric and adult subjects (median age = 4.6 years, range = 3 weeks to 90 years) with lens status phakic, aphakic, or pseudophakic (n = 33, 7, and 21 subjects, respectively). Following transfer learning, a pretrained Densenet-121 model was fine-tuned on these images. Metrics were calculated for testing dataset results aggregated from fivefold cross-validation. For each fold, 20% of total subjects were partitioned for testing and the remaining subjects were used for training and validation (80:20 split). Results: Our neural network trained across 60 epochs achieved recall 96.15%, precision 96.14%, F1-score 96.14%, false positive rate 3.74%, and area under the curve (AUC) 0.992. Feature saliency heatmaps consistently involved the lens. Algorithm performance was compared using 2 image sets, 1 from subjects of all ages, and the second from only subjects under age 10 years, with similar performance under both circumstances. Conclusions: A neural network trained on a relatively small UBM image set classified lens status with satisfactory recall and precision. Adult and pediatric image sets offered roughly equivalent performance. Future studies will explore automated UBM image classification for complex anterior segment pathology. Translational Relevance: Deep learning models can evaluate lens status from UBM images in adult and pediatric subjects using a limited image set.

RESUMO

AML1-ETO (AE) is a fusion transcription factor, generated by the t(8;21) translocation, that functions as a leukemia promoting oncogene. Here, we demonstrate that TATA-Box Binding Protein Associated Factor 1 (TAF1) associates with K43 acetylated AE and this association plays a pivotal role in the proliferation of AE-expressing acute myeloid leukemia (AML) cells. ChIP-sequencing indicates significant overlap of the TAF1 and AE binding sites. Knockdown of TAF1 alters the association of AE with chromatin, affecting of the expression of genes that are activated or repressed by AE. Furthermore, TAF1 is required for leukemic cell self-renewal and its reduction promotes the differentiation and apoptosis of AE+ AML cells, thereby impairing AE driven leukemogenesis. Together, our findings reveal a role of TAF1 in leukemogenesis and identify TAF1 as a potential therapeutic target for AE-expressing leukemia.

La epigenética es el estudio de los cambios de los genes y su expresión que no generan modificaciones en la secuencia de ADN. Esta implica una serie de mecanismos como la metilación de citosinas del ADN, lo cual se manifiesta en el fenotipo y genera heredabilidad, también llamada heren-cia "suave y fuerte". Aquí es importante recordar que el fenotipo es una condición emergente del genotipo y de la epigenética. La epigenética mantiene una correcta impronta genómica en condiciones naturales y sin perturbaciones, por el contrario, anormalidades epigenéticas generan una expresión génica y fenotípica inapropiada. Las modifica-ciones fenotípicas se presentan en plantas, animales y seres humanos, lo que finalmente se expresa en los ecosistemas generando una condición emergente más allá de la condición humana: la salud. De acuerdo a lo anterior y desde la perspectiva del pensamiento complejo, la salud es la expresión de la vida per se, razón por la cual la epigenética forma parte de los factores que permiten la emergencia de la salud. En otras palabras, la salud surge como consecuencia de la interacción entre genética, cultura, sociedad, economía, pensamiento, vivencias y experiencias, y es el reflejo de lo que nos hace plenamente humanos.


Epigenetics refers to the study of gene-changes and its expression without altering DNA sequencing. It implies a series of mechanisms such as DNA cytokine methylation. Such mechanism generates heritability and is expressed in the phenotype. Heritability has also become known as soft and strong heredity. The phenotype is an emergent condition stemming from both the genotype and Epigenetics, thus responding to the four dimensions of biological evolution. Under natural, undisturbed conditions, Epigenetics maintains an appropriate genomic imprinting, while epigenetic abnormalities create gene and phenotypic inappropriate expressions. Phenotypic modifications are also present in plants, animals and human beings. Such, ultimately is expressed in ecosystems generating an emerging condition that stretches beyond the human condition and constitutes that which is referred to as health. According to the complex thought approach, health goes beyond determination and balance; health is the very expression of life per se. It emerges as a consequence of genetics, economics, thought and experiences, encompassing that which reflects what makes us fully humans


Epigenética é o estudo das alterações genéticas e de sua expressão sem gerar modificações na sequência do DNA. Envolve uma série de mecanismos, como a metilação da DNA pela citosina, que se manifesta no fenótipo, gerando herdabilidade, denominada herança "mole e forte". O fenótipo é uma condição emergen-te do genótipo e, por sua vez, da epigenética, respondendo às quatro dimensões da evolução biológica. A epigenética em condições naturais, sem distúrbios, mantém uma impressão genómica certa, ao contrário, as anormalidades epigenéticas geram um gene inadequado e expressão fenotípica. Da mesma forma, modificações fenotípicas são apresentadas em plantas, animais e seres humanos, que são finalmente expressas em ecossistemas, gerando uma condição emergente além da condição humana: Saúde. De acordo com o exposto e sob a perspectiva do pensamento complexo, a saúde vai além da determinação, do equilíbrio, sendo a expressão da vida em si. A epigenética é uma dimensão que infere na emergência, mas não um fator determinístico. Finalmente, a saúde surge como conseqüência da genética, da cultura, da condição social e econômica, do pensamento, em fim, das experiências, refletindo o que nos torna plenamente humanos.

Background On October 1, 2015, the Center for Medicare and Medicaid Services transitioned from the International Classification of Diseases, Ninth Revision ( ICD-9) to the ICD, Tenth Revision ( ICD-10) compendium of codes for diagnosis and billing in health care, but translation between the two is often inexact. Here we describe a validated crosswalk to translate ICD-9 codes into ICD-10 codes, with a focus on complications after carotid revascularization and endovascular aortic aneurysm repair. Methods and Results We devised an 8-step process to derive and validate ICD-10 codes from existing ICD-9 codes. We used publicly available sources, including the General Equivalence Mapping database, to translate ICD-9 codes used in prior work to ICD-10 codes. We defined ICD-10 codes as validated if they were concordant with the initial ICD-9 codes after manual comparison by two physicians. Our primary validation measure was the percent of valid ICD-10 codes out of the total ICD-10 codes obtained during translation. We began with 126 ICD-9 diagnosis codes used for complication identification after carotid revascularization procedures, and 97 ICD-9 codes for complications after endovascular aortic aneurysm procedures. Translation generated 143 ICD-10 codes for carotid revascularization, a 14% increase from the initial 126 codes. Manual comparison demonstrated 98% concordance, with 99% agreement between the reviewers. Similarly, we identified 108 ICD-10 codes for endovascular aortic aneurysm repair, an 11% increase from the initial 97 ICD-9 codes. We again noted excellent concordance and agreement (98% and 100%, respectively). Manual review identified 4 ICD-10 codes incorrectly translated from ICD-9 codes for carotid revascularization, and 3 codes incorrectly translated for endovascular aortic aneurysm repair. Conclusions Algorithms to crosswalk lists of ICD-9 codes to ICD-10 codes can leverage electronic resources to minimize the burden of code translation. However, manual review for code validation may be necessary, with collaboration across institutions for researchers to share their efforts.

Introducción: frecuentemente la ingesta alimentaria de las personas no coincide con las recomendaciones de salud. Objetivos: comparar la ingesta alimentaria de la población estudiada con las recomendaciones de las guías nacionales como base para definir los contenidos del plan de educación nutricional de un programa de prevención primaria de diabetes. Materiales y métodos: evaluamos la ingesta alimentaria de personas de 45 a 75 años, atendidas en el sector público de salud de La Plata y Berisso, entre marzo de 2015 y marzo de 2016. Utilizamos una encuesta de ingesta de alimentos estructurada y autoadministrada basada en un listado de alimentos de consumo habitual de nuestra población (NutriQuid). Los participantes completaron la encuesta durante dos días hábiles y un día de fin de semana. Verificamos el porcentaje de adecuación de la ingesta energética y nutrientes a las recomendaciones nutricionales del Ministerio de Salud de la Nación. En un subgrupo analizamos la relación entre el índice de masa corporal e ingesta energética y el consumo de sodio en hipertensos/normotensos. La evaluación estadística incluyó ANOVA y el test de t. Resultados: evaluamos las encuestas realizadas a 142 personas: 50% o más de la población encuestada siguió un plan de alimentación que no cumple con las recomendaciones del Ministerio de Salud de la Nación respecto de energía, proteínas, lípidos, sodio, ácidos grasos saturados, colesterol y azúcares refinados. Más del 80% de los participantes consumió cantidades insuficientes de fibra, potasio, frutas y verduras. Sólo un 27% de los hipertensos y un 14% de los normotensos consumieron las cantidades de sodio recomendadas. El 35% de los hipertensos y el 48% de los normotensos declararon consumir más del doble del valor recomendado. Conclusiones: consideramos que nuestros resultados apoyan la iniciativa de implementar un programa de educación alimentaria basado en las recomendaciones nutricionales.
